What it is

Muscle pain (myalgia) describes pain originating from muscle tissue — experienced as aching, soreness, tenderness, or deep dull pain within one or more muscles.

Patterns of Muscle Pain

Myalgia (muscle pain) encompasses a spectrum from the familiar delayed onset muscle soreness (DOMS) following unaccustomed exercise, to the widespread, persistent muscle pain of fibromyalgia, to the acute diffuse myalgia of viral infection (influenza, COVID-19), to the proximal limb pain and weakness of inflammatory myopathy (polymyositis, dermatomyositis). Distribution, onset, character, and associated features guide differential diagnosis. Acute localised myalgia commonly reflects strain, overuse, or muscle contusion. Diffuse myalgia with systemic features (fever, fatigue) suggests viral or systemic illness. Persistent widespread myalgia with fatigue and sleep disturbance is characteristic of fibromyalgia. Medication-induced myopathy (particularly statins — in 5–10% of users) is an important and underrecognised cause. Proximal muscle weakness with pain raises concern for inflammatory myopathy.
