Reduced alertness describes a diminished state of wakefulness and cognitive readiness — a lower-than-normal level of responsiveness to the environment, slower reaction times, impaired vigilance, and a tendency toward drowsiness during activities requiring sustained attention. It can result from sleep deprivation, sleep disorders (including sleep apnoea), sedating medications, alcohol and substance use, hypoglycaemia, hypothyroidism, anaemia, depression, and serious neurological conditions. Reduced alertness during driving or operating machinery is a significant safety concern. Paradoxically, chronic stimulant use (caffeine dependence) can produce reduced baseline alertness as the brain downregulates its own arousal systems in response to sustained artificial stimulation — making cessation temporarily worsens alertness before improving it.
