COPD (chronic obstructive pulmonary disease) produces persistent expiratory wheeze from fixed airway obstruction and mucus hypersecretion. New onset wheeze in an adult without prior asthma warrants investigation to exclude obstruction and cardiac causes. In infants, bronchiolitis (RSV) produces characteristic wheeze.
